Heart Failure - Homecare Overview


The focus of chronic disease management in the home health setting is the prevention of exacerbations and the avoidance of unnecessary hospitalization and emergency room visits.

Specific components of a successful heart failure disease management program include the use of evidence-based practice guidelines, patient self-management education, early recognition and attention to signs and symptoms of fluid overload and collaboration with physicians and other healthcare providers.
