All 'Improvers' Welcome

The healthcare system is fragmented. As a result of poor care coordination, patients suffer.

To join others to improve care, join the CareTrek Learning and Action Network (LAN). The LAN is an opportunity to:

  • Be part of the national movement to improve care transitions and patient safety
  • Share your expertise and perspective with other providers
  • Access free resources, tools and education on interventions that have been proven to work
  • Build relationships with other healthcare professionals in your community

    Only LAN members will have access to resources, education and tools.

      In developing the LAN, CIMRO of Nebraska will be recruiting community leaders, stakeholders, providers and patients in all parts of Nebraska to form and lead regional community-based groups. LAN members will work together to implement sustainable evidence-based resources and interventions to reduce hospital readmissions by 20 percent statewide.

        Participation in the LAN is free.

          LAN members will have access to free ongoing education. A variety of mediums will be utilized. Activities may include WebEx sessions, recorded learning sessions, conference calls and written information. Presenters will include national leaders, intervention experts, CIMRO of Nebraska staff and Nebraska CareTrek LAN members.

The Triple AIM

The healthcare challenges in Nebraska require a statewide, concerted effort to achieve the national Triple Aim of improving overall health, the experience of care and the affordability of care. CareTrek plans to meet each of these AIMs by:

Overall Health Improvement 

  • Through improved care coordination, prevent avoidable hospital readmissions within 30 days of discharge, thus avoiding unnecessary rehospitalizations and associated burdens
  • Reduce the overall readmission rate in Nebraska by 20 percent over three years

Care Experience

  • Empower patients and families to better understand their healthcare needs and follow-up care
  • Make information more easily accessible and in a format that is understood
  • Improve knowledge of and access to community resources

Affordability

  • Decrease healthcare costs by avoiding preventable readmissions
