The goal of this national Homecare Quality Initiative is to reduce avoidable hospitalizations by implementing best practices and utilizing care delivery systems to prevent deterioration in the health status of patients. Improvement efforts should focus on coordination of care between healthcare providers, identifying patients at high risk for re-hospitalization, stabilizing and managing chronic health conditions and promoting self-management skills for patients and caregivers.

The Polish Your Practice Series Toolbox (located below in the documents section) is a list of resources and tools that will assist clinicians in brushing up on current guidelines and practices for heart failure, wound management and diabetes, as well as health literacy, the chronic care model, working with Medicare Advantage plans and data analysis.  The resources include recorded WebEx presentations with accompanying handouts and self-study post-tests for each clinical topic, as well as links to all of the ZONE (Stoplight) tools.
