The goal of this national Homecare Quality Initiative is to reduce avoidable hospitalizations by implementing
best practices and utilizing care delivery systems to prevent deterioration in
the health status of patients. Improvement efforts should focus on coordination
of care between healthcare providers, identifying patients at high risk for
re-hospitalization, stabilizing and managing chronic health conditions and
promoting self-management skills for patients and caregivers.
