COPD Overview


The focus of chronic disease management in the home health setting is the prevention of exacerbations and the avoidance of unnecessary hospitalization and emergency room visits.

Specific components of a successful Chronic Obstructive Pulmonary Disease (COPD) disease management program include the use of evidence-based practice guidelines, patient self-management education, early recognition and attention to changes in the level of dyspnea, and collaboration with physicians and other healthcare providers.
