Telehealth refers to the remote delivery of care and monitoring of patients
by the homecare agency. Two types of telehealth options available include phone monitoring
and telemonitoring. Phone monitoring consists of scheduled patient encounters using
the telephone. Telemonitoring uses electronic devices to collect and transmit
clinical data. Utilization of a telehealth system is a valuable tool to assist
agencies in reducing avoidable acute care hospitalizations.
